Description

The Case Manager is responsible for coordinating patient care across the continuum, ensuring safe, timely, and cost-effective treatment. This role involves patient assessment, discharge planning, collaboration with interdisciplinary teams, and coordination of post-acute services while advocating for patient needs and compliance with care plans.
Requirements:

Active RN or MSW license (as required by the facility/state)
Minimum 1–2 years of recent case management experience (acute care preferred)
Strong knowledge of discharge planning and care coordination
Excellent communication and documentation skills
Familiarity with utilization review and payer guidelines preferred

Aaky J was very responsive, respectful and considerate, he responds quickly and he returns texts asap, I’ve even gotten random texts just asking how I was doing. I’ve never worked with Lancesoft before but Aaky has made it comfortable.

Cons

Maybe the only thing which truly isn’t a problem is the dialect, but we got through that as well.
