We’re seeking an experienced Registered Nurse (RN) for a Home Health / Hospice assignment with Marshfield Medical Center – Beaver Dam. This is a great opportunity for a compassionate RN who thrives in post-acute care and enjoys building meaningful patient relationships in the home setting.

Position Details

• Unit: Home Health / Hospice RN
• Care Setting: Post-Acute
• Shift: Days | 8-hour shifts
• Schedule: Monday–Friday (8:00 AM–4:30 PM, may vary)
• Assignment Length: 13 weeks
• Guaranteed Hours: 40 hours/week
• Charting System: Cerner
• Orientation: 8 hours + 2–5 days field training (based on experience)
• Floating: No unit floating, but will perform both home health & hospice visits

Patient Population

• Adult & Geriatric patients (no pediatrics)
• Common diagnoses include CHF, COPD, dementia/Alzheimer’s, stroke, cancer, diabetes, wound care, post-surgical care, and chronic disease management

Requirements

• Active RN license (license required to submit)
• 2+ year of RN experience
• BLS (AHA) required
• OASIS experience required
• Travel experience preferred
• Strong skills in:
  • Chronic disease management
  • Patient & family education
  • IV administration & central line care
  • NPWT, ostomy & complex wound care
  • Hospice care, pain & symptom management, end-of-life support

Additional Expectations

• 5–6 patients per day (25–30 points/week)
• Daily huddle calls, weekly IDT meetings, monthly staff meetings
• On-call: 1 week/weekend every 6 weeks
• Holidays: 1 major & 1 minor required
• Weekends only during on-call rotation

Dress Code: Solid color scrubs (no patterns/logos unless company-issued), closed-toe non-slip shoes

⚠️ Must disclose any prior employment with Marshfield Medical Center (must be 6 months since end date). All clinicians must clock in/out at the facility.
