Travel RN – Case Manager Location: Mission Hills, CA Contract: 13 Weeks Shift: Day 5x8 (08:00 AM – 4:30 PM)Unit: Case Management  Key Responsibilities<ul><li>Perform care coordination and discharge planning for acute hospital patients</li><li>Apply InterQual criteria for utilization review and admission decisions</li><li>Develop and manage individualized plans of care</li><li>Coordinate DME, SNF placements, and ambulance transportation independently</li><li>Collaborate with social services, transportation, providers, and families</li><li>Participate in family meetings and interdisciplinary care planning</li><li>Navigate CMS, DRG, HMO, IPA requirements and payer guidelines</li><li>Support patients with mental health history, substance use, homelessness, and crisis needs</li><li>Ensure compliance with regulatory and hospital policies</li><li>Document all care activities using Epic and computerized systems</li></ul>Required QualificationsActive CA RN License (Pending accepted but must be active by start)  2+ years Acute Care Hospital Case Manager experience  Strong experience in Care Coordination &amp; Discharge Planning Proficiency in InterQual Criteria and Utilization Review Epic charting experience Prior travel experience preferred (1st-time travelers accepted) Behavioral Health–only experience will NOT be considered Hospital Case Management experience is REQUIRED
