Description
Registered Nurse (RN) – Case Manager
Anchorage, Alaska
Start Date: ASAP | Duration: 13 Weeks
Shift: Day Shift — 5x8 Hours
Key Responsibilities
- Coordinate patient care and discharge planning across multidisciplinary teams
- Apply Milliman Guidelines for utilization review and case management
- Conduct patient needs assessments and develop individualized care plans
- Manage DME orders, care transitions, and follow-up planning
- Ensure compliance with CMS regulations and HIPAA guidelines
- Collaborate with physicians, social workers, and ancillary services
- Maintain accurate clinical documentation in Epic
- Support patient education, advocacy, and continuity of care
Required Qualifications
- Active Alaska RN License (Required – must be in hand for Auto Offer)
- BLS Certification
- Minimum 2 years Acute Care RN Experience
- Epic EMR Experience (Required)
- Strong Case Management / Utilization Review Experience
- ACM Certification (Preferred)
- Behavioral Health Experience will NOT qualify
