Job Title: Physical Therapist (PT) – Home Health
Job Type: Contract
Duration: 13 Weeks
Urgency: Quick Start
Schedule: 40 Hours per Week (Saturday–Wednesday)
Shift: Days

Position Overview

We are seeking a licensed Physical Therapist (PT) for a Home Health contract assignment covering Solano County, California. This role focuses on providing high-quality in-home physical therapy services while managing start-of-care visits, supervising PTAs, and ensuring timely documentation and communication.

Key Responsibilities

  • Prioritize and complete start-of-care (SOC) visits
  • Perform comprehensive patient evaluations and develop individualized treatment plans
  • Supervise and provide clinical oversight to Physical Therapy Assistants (PTAs)
  • Complete mandatory next-day reporting for SOC visits
  • Submit clinical notes and documentation within 48 hours of each visit
  • Maintain timely communication with supervisors, intake teams, and office staff
  • Remain responsive during normal business hours
  • Manage assigned home health caseload and visit mix to meet productivity standards

Productivity Expectations

  • 30 productivity points per week
  • Point-to-hour conversion: 1 point = 1.33 hours
  • Caseload structured to average 8 hours per day / 40 hours per week
  • Overtime requires prior approval from the Clinical Supervisor

Visit Types & Point Values

  • Start of Care: 2 points
  • Recertification, Resumption of Care, OASIS Discharge, Evaluation: 1.5 points
  • Revisit / Non-OASIS Discharge: 1 point

Territory & Travel

  • Primary coverage area: Solano County, CA (territory expansion as needed)
  • Mileage reimbursement provided from first to last patient
  • Parking and toll reimbursements available
  • Equipment issued: iPad and mobile communication tools

Systems & Training

  • Documentation system: Kinnser
  • Training: 3 days (1 in-house day, 2 field days)

Qualifications & Requirements

  • Active California Physical Therapist license (must be current)
  • Minimum of 1 year of Home Health experience
  • Strong clinical assessment, documentation, and time-management skills
  • Ability to work independently in a home-based care environment
