Description
Title: Physical Therapist – Home Health
Location: Saint Joseph, MO
Duration: 13 Weeks
Shift: Day Shift – 5x8-Hour (8:00 AM – 4:30 PM); Call required 1 Sunday every 5 weeks; Every 5th week work Saturday & Sunday for admits (2 weekdays off that week)
Job Responsibilities:
- Provide Home Health Physical Therapy services to adult and geriatric patients
- Perform Start of Care (SOC), admissions, evaluations, reassessments, and discharges (PTs complete their own SOC)
- Manage approximately 6 patient visits per day (adjusted when travel distance is high)
- Provide care across a 13-county coverage area; rotation to furthest counties weekly
- Develop and implement individualized treatment plans and home exercise programs
- Complete OASIS documentation and maintain accurate records in Epic (required)
- Perform assessments including strength testing, pain assessment, sensory response, vital sign monitoring, skin integrity, environmental barriers, body mechanics, ROM (AROM/AAROM/PROM), adaptive/assistive device assessment
- Establish short- and long-term goals, plan of care, RUG levels, and discharge planning
- Provide patient and family education including safety, injury prevention, medications, and abuse recognition
- Collaborate with interdisciplinary team including PTs, social services, interpretation services, and unit secretary
- PTA staff perform routine visits; PT oversees plan of care
Required Qualifications:
- DPT (Doctor of Physical Therapy)
- BLS Certification
- Active State License (pending license accepted at submission; must be in hand prior to start)
- Minimum 1 year PT experience
- Must have Home Health experience
- Epic charting experience (required)
- Travel experience preferred
- Ability to participate in weekend and call rotation
Required Clinical Experience & Skills:
- Orthopedics (total knee replacement, hip fractures/injuries, low back pain, osteoarthritis, DDD, DJD, sciatica)
- Gait training and extremity mobilization
- Post-stroke rehabilitation
- Dementia/Alzheimer’s and cognitive disorders
- Use of assistive devices (rolling walker, standard walker, crutches, gait belt, sliding boards, sit-to-stand lift, hospital beds, lower extremity prosthetics, supplemental oxygen)
- OASIS documentation
- Home Health documentation standards
Preferred Skills & Experience:
- Geriatrics, Neurology, Oncology, Sports Medicine, Wound Care
- Cardiopulmonary conditions: COPD, CHF, CABG, Emphysema, PVD
- Neurologic conditions: Parkinson’s disease, spinal cord injury, traumatic brain injury, multiple sclerosis, muscular dystrophy, nerve palsy, concussion, NDT
- Orthopedic specialties: pelvic floor training, post-op cervical/shoulder/spinal surgery, prosthetic training, spinal mobilization, taping
- Equipment: Hoyer lift, wheelchair assessment, specialty beds, shoulder slings, soft/hard collars
- Modalities: TENS, E-stim, ultrasound, moist heat
- Understanding of payor requirements
