✔ Minimum 2 years acute inpatient case management experience
✔ Recent Acute Hospital Case Management
✔ ER Department Case Management experience
✔ Recent Discharge Planning experience
✔ BLS Certification
✔ Active California RN License (in hand at submission)
✔ 2 Supervisor References (within past 1 year)
