Job Title: Physical Therapist (PT) – Home Health
Job Type: Contract

Duration: 13 Weeks
Schedule: 40 hours per week, Monday–Friday, plus one weekend per month rotation

Position Overview

We are seeking a licensed Physical Therapist with home health experience to provide high-quality patient care in a home-based setting. The ideal candidate is organized, responsive, and comfortable managing documentation and communication in a fast-paced environment.

Key Responsibilities

• Prioritize and complete Start of Care visits in a timely manner
• Supervise Physical Therapy Assistants (PTAs)
• Submit mandatory next-day reporting for Start of Care visits
• Complete clinical notes and documentation within 48 hours of each visit
• Maintain timely communication with supervisor, intake team, and office staff
• Remain responsive during normal business hours
• Manage an average caseload equivalent to 40 hours per week
• Obtain pre-approval for any overtime from the Clinical Supervisor

Productivity Expectations

• 30 productivity points per week required
• 1 point = 1.33 hours
• Caseload and visit mix average 8 hours per day / 40 hours per week

Visit Types & Point Values:

• Start of Care – 2 points
• Recertification / Resumption of Care / OASIS Discharge / Evaluation – 1.5 points
• Revisit / Non-OASIS Discharge – 1 point

Territory

• Assigned coverage area within California
• Territory may expand based on patient care needs

Requirements

• Active California Physical Therapist license (must be currently active)
• Minimum of 1 year of Home Health experience
• Strong clinical documentation skills
• Ability to work independently in a field-based setting

Training & Support

• 3 full days of training (1 in-house, 2 in the field)
• Documentation system: Kinnser
• Equipment provided: iPad and Verizon One Talk
