Description

This is a home hospice position. Travelers will cover Fond du Lac through Oshkosh to Appleton. Most of the patients are in the Oshkosh city area.

Patient Types/Common Diagnoses:
o Adults only - End of Life patients

Shift times: Monday - Friday, 0800-1630

On call? One call shift a month (2000-0800)

Minimum Years of Experience required? 2 years of Hospice experience needed

BLS

Skills required:
o End of life care/planning, wound assessments, care coordination, pain/symptom management, triage
o EPIC Remote experience required

Technology/Equipment:
o EMR: EPIC Remote Client - required
o IV Pump: Sapphire
