Description

Description:

Coordinates with physicians, staff and non-Kaiser providers and facilities regarding patients transitions and discharge planning. In conjunction with physicians, develops plans of care and discharge plans, and makes recommendations for alternative levels of care as required.

Role Highlights:

• Acute Care Case Management
• Utilization Review & Care Coordination
• Discharge Planning & Patient Transitions
• Work closely with physicians & multidisciplinary teams

✅ Must-Haves:
• Hospital/Inpatient experience
• Acute care department background
• Strong discharge planning experience
• Minimum 2 years in Case Management/UM

📌 Requirements:

• Active California RN License
• BLS Certification

⭐ Preferred: BSN / Masters in Case Management
