Description

Role Highlights:

Acute Care Case Management
Utilization Review & Care Coordination
Discharge Planning & Patient Transitions
Work closely with physicians & multidisciplinary teams

Must-Haves:

Hospital/Inpatient experience
Acute care department background
Strong discharge planning experience
Minimum 2 years in Case Management/UM

Requirements:

Active California RN License
BLS Certification

Additional Requirements • Knowledge of regulatory agencies and standards including: TJC, DMHC, CMS, NCQA, HIPAA, ERISA, EMTALA • Strong communication, customer service, and critical thinking skills • Excellent clinical judgment and problem-solving ability • Proficiency with word processing, computer navigation, and analytic data use • Understanding of healthcare benefits across various business lines • Ability to work in a Labor/Management Partnership environment
