RN CASE MANAGER- CONTRACT TO HIRE- 4x10s - CHARLESTON SC
PAY: $35-39/HR
MUST HAVES:
-Current RN license in South Carolina or compact.
-Bachelor's degree in Nursing (BSN) required
-Minimum of 3 years of clinical nursing experience, with at least 1 year in case management or discharge planning.
-BLS (AHA)
-Strong knowledge of healthcare systems, discharge planning, and community resources.
-Excellent communication, organizational, and problem-solving skills.
-Ability to work effectively in a fast-paced, multidisciplinary environment.
JOB DESCRIPTION
The RN Case Manager is responsible for coordinating patient care and ensuring effective disposition planning within the hospital. This role involves assessing patient needs, developing care plans, and collaborating with healthcare providers to facilitate timely and appropriate discharge planning. The RN Case Manager will work closely with patients, families, and interdisciplinary teams to optimize patient outcomes and ensure continuity of care. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system.
