Title: Physical Therapist (PT) – Home Health

Location: Port Charlotte, FL 33948 (Coverage area: Port Charlotte, Englewood, Venice, Nokomis, Osprey, Sarasota – up to 60-mile radius from hub)

Duration: 13 weeks

Shift: Day – 5x8-hour shifts (8:00 AM – 5:00 PM)
 

Job Responsibilities:

• Provide in-home physical therapy services to geriatric patients.
• Manage an average of 5–7 patients per day.
• Perform evaluations, develop care plans, and deliver skilled PT interventions.
• Complete OASIS documentation (required for clinicians performing SOCs).
• Document patient care in HCHB EMR (required).
• Travel throughout assigned territory (up to 60-mile radius).
• Maintain flexibility with territory coverage as needed.
• Participate in home health and/or hospice care as applicable.
• Ensure timely documentation and compliance with Medicare home health standards.

Required Skills & Qualifications:

• Minimum 1 year of recent Home Health experience.
• OASIS experience required (for SOC clinicians).
• HCHB EMR experience required.
• Active state PT license in hand (license pending accepted for therapy only).
• BLS certification required.
• Ability to manage geriatric caseload independently.
• Proof of vehicle insurance required.
• Background check hits not accepted.
• Compliance items must be initiated within 24 hours of offer acceptance.
• RTO must be disclosed at time of submission.
• Home Health and/or Hospice competency checklist required.
• Traveler must be flexible with territory coverage.
• On-call may be required.
• First-time travelers accepted.
