Title: Registered Nurse (RN) – Home Health
Location: Clear Lake, CA (Lake County Coverage Area)
Position Type: 10 Weeks (40 Hours Expected)
Shift: Day – 5x8 Hour Shifts (8:00 AM – 4:30 PM)

Job Description:
• Certifications: Active California RN License required
• Experience: Home Health experience required; Minimum 1+ year preferred
• Required Skills: OASIS experience required; HCHB (Homecare Homebase) software experience required
• Setting: Home Health (Not Hospital-Based)
• Schedule: 40 hours per week; Day shift
• Coverage Area: Lake County, CA
• Mileage: Government rate reimbursement provided for patient travel
• Responsibilities:

Perform in-home patient assessments and skilled nursing visits
Complete OASIS documentation accurately and timely
Utilize HCHB for charting and care coordination
Develop and implement individualized care plans
Collaborate with interdisciplinary team to ensure continuity of care
