Description

Travel Registered Nurse (RN) – Case Manager

Apple Valley, CA

Duration: 13 Weeks

Shift: Days – 5x8s (8:00 AM – 4:30 PM)
Weekend Rotation Required (every other weekend)

Important Requirements

Seasoned traveler required (HARDSTOP – no first-time travelers)
Must have 2+ years Acute Care Case Management / Discharge Planning
Behavioral health–only experience does NOT qualify
Active CA RN license (pending accepted but must be active by start date)
Epic experience required

Position Overview

Seeking an experienced Acute Care RN Case Manager to manage a caseload of up to 25 patients in a hospital setting. This role focuses on care coordination, utilization review, and complex discharge planning across diverse patient populations.

Core Responsibilities

Patient assessment & needs evaluation
Care coordination & discharge planning
Utilization review & admission criteria
Participate in treatment plan development
Organize & lead family meetings
Arrange DME, SNF placement & ambulance transport independently
Prior authorizations & benefits eligibility review
Disease management & plan of care oversight
Regulatory compliance (CMS, DRGs, Joint Commission standards)
Document in Epic EMR

Required Experience

✔ Acute hospital case management experience (ED, ICU, MS, Surgical Services)
✔ Utilization Review
✔ Knowledge of HMOs, IPAs
✔ Familiarity with:

Adoption/foster care processes
Substance abuse treatment resources
Homeless patient discharge coordination
Mental health crisis support
Advance directives & POLST
Protective services processes
