Start Date: March 23, 2026
Assignment Length: 13 Weeks
Pay: $65 per hour (Hourly pay — not per visit)
Schedule: Monday–Friday, Full-Time
Territory: Center City & North Philadelphia (Field-Based)
About the Role
We are seeking an experienced Home Health Registered Nurse (RN) for a 13‑week assignment covering Center City and North Philadelphia. This role requires a clinician skilled in high-volume patient care, detailed clinical documentation, and strong OASIS proficiency.
You will complete daily field visits, manage complex clinical needs, and collaborate closely with the home health team to ensure patients receive exceptional care at home.
Key Responsibilities
- Perform skilled nursing visits in patients’ homes across Center City & North Philadelphia
- Conduct Start of Care (SOC) assessments
- Complete OASIS documentation accurately and on time
- Provide wound care, palliative care, and discharge planning
- Manage a daily caseload of 5–6 patients
- Deliver patient and family education
- Maintain thorough and timely clinical documentation
- Communicate patient changes promptly to the interdisciplinary team
- Drive frequently throughout the assigned territory
Required Qualifications
- Active Pennsylvania RN License
- 2+ years of recent Home Health experience
- Ability to manage multiple patients per day in a fast‑paced home health setting
- Recent, hands‑on OASIS experience
- Experience with SOC, wound care, palliative care, and discharge planning
- Must live near Center City or North Philadelphia to support same‑day field coverage
- Comfortable with heavy driving and high-volume charting
- Strong assessment and time‑management skills
