Position Overview
Seeking an experienced Acute Care RN Case Manager with strong discharge planning and utilization review background. Candidate must be comfortable working independently, coordinating complex discharges, and applying InterQual criteria.
Acute behavioral health-only experience will not meet acute hospital requirements.

Requirements
โ Active California RN license (pending accepted but must be active by start)
โ 2+ years acute hospital case management/discharge planning experience
โ Travel experience required (first-time travelers may be screened)
โ Epic experience required
โ Strong knowledge of Medicare guidelines

Key Responsibilities
- Perform admission reviews using InterQual (ED, Mother Baby, NICU)
- Determine medical necessity per evidence-based guidelines
- Conduct care coordination and discharge planning
- Arrange DME, SNF placements, and transport
- Coordinate family meetings and interdisciplinary planning
- Manage complex social cases (homelessness, mental health crisis, substance abuse, foster/adoption systems)
- Ensure regulatory compliance (CMS, HIPAA, DRG guidelines)
- Weekend coverage focusing on discharge planning (may oversee up to 2 units)
