Job Title: Physical Therapist (PT) – Home Health
Job Type: Contract

Schedule: 40 Hours per Week | Saturday – Wednesday

Position Urgency: Quick Start

Position Overview:

We are seeking an experienced Physical Therapist (PT) for a Home Health contract assignment serving patients throughout Solano County, California. The ideal candidate will provide high-quality, patient-centered care in the home setting while maintaining strong communication and timely documentation standards.

Key Responsibilities:

  • Prioritize and complete Start of Care (SOC) visits
  • Perform evaluations and develop individualized treatment plans
  • Supervise Physical Therapy Assistants (PTAs)
  • Complete mandatory next-day reporting for Start of Care visits
  • Submit clinical documentation within 48 hours of each visit
  • Maintain consistent communication with supervisors, intake, and office staff
  • Remain responsive during normal business hours
  • Expand coverage territory as needed

Productivity Requirements:

  • 30 points per week
  • Point-to-hour conversion: 1 point = 1.33 hours
  • Caseload averages 8 hours per day / 40 hours per week
  • Overtime must be pre-approved by the Clinical Supervisor

Visit Point System:

  • Start of Care: 2 points
  • Recertification / Resumption of Care / OASIS Discharge / Evaluation: 1.5 points
  • Revisit / Non-OASIS Discharge: 1 point

Training & Equipment:

  • 3 days of training (1 in-house, 2 in the field)
  • iPad and communication device provided
  • Mileage reimbursement (from first to last patient visit)
  • Parking and toll reimbursement available
  • Company vehicle option available

Qualifications:

  • Active California Physical Therapist license
  • Minimum of 1 year of Home Health experience required
  • Strong clinical assessment and care planning skills
  • Experience with electronic documentation systems (Kinnser preferred)
  • Excellent communication and organizational skills

  • Territory-based position within Solano County (coverage may expand as needed)
  • Quick start opportunity
  • No holiday coverage required
