Need Specifics: Travel Pediatric RN Case Manager – Hospice Fresno, CA Hinds Hospice – Pediatric Hospice / Concurrent Care Start: ASAP 4–6 Week Contract 40 hrs/week | M–F 8:00a–4:30p Role: Pediatric RN Case Manager providing in-home hospice and concurrent care services for patients birth–21 years. Responsible for care coordination, skilled nursing assessments, symptom management, and family education. 4–5 home visits per day. Details: On-call approx. 7 days/month 1:15 caseload ratio Charting: NetSmart Requirements: Active CA RN License Minimum 2 years pediatric experience (PICU, NICU, pediatric acute care, oncology, and/or hospice) Hospice or case management experience preferred BLS required Reliable transportation Valid driver’s license Current auto insurance with acceptable coverage Current vehicle registration Submission Notes: Resume must clearly show pediatric experience Hospice and/or case management experience must be detailed Active CA license and BLS must be attached Proof of driver’s license and auto insurance required Include availability and start date Confirm candidate is comfortable with home visits and on-call rotation
