Travel Nurse Opportunity – RN Case Manager

📍 Santa Clara, CA

📅 13-Week Contract

Join a patient-centered care coordination team where your clinical expertise and advocacy will help guide patients through complex healthcare journeys. This assignment is ideal for an organized and compassionate RN Case Manager who excels at coordinating care across multiple disciplines and ensuring patients receive timely, appropriate services.

Position Overview:
In this role, you will assess patient needs, develop individualized care plans, and coordinate services across inpatient, outpatient, and community settings. You will review clinical information, monitor progress toward care goals, and collaborate with physicians, therapists, social workers, and insurance providers to remove barriers to care.

You will also educate patients and families on treatment plans, available resources, and follow-up care to promote long-term health and stability. This role requires strong clinical judgment, excellent communication skills, and a commitment to patient advocacy.

Requirements:
• Active RN license
• BLS certification (AHA)
• Minimum 1 year of case management or related nursing experience
