Travel Case Manager Opportunity – Transitions Program

Join a patient-centered care coordination team where your expertise directly improves patient outcomes and healthcare navigation. This remote case management role focuses on short-term telephonic coordination, discharge planning, and patient advocacy—ideal for a professional who thrives in communication-driven care coordination and utilization review.

Position Details:
Start Date: (available per client need)
Schedule: Telephonic case management with rotating weekend coverage every 7 weeks
Caseload: 25–35+ patients managed over 30-day episodes with weekly follow-up calls
Work Setting: Remote after in-office orientation (laptop and phone provided)

Key Responsibilities:

  • Conduct telephonic case management and care coordination
  • Develop individualized care plans and monitor patient progress
  • Coordinate multidisciplinary services and discharge planning
  • Educate patients and families on treatment plans and available resources
  • Advocate for patient needs and remove barriers to care
  • Collaborate with healthcare teams to ensure seamless transitions

Role Overview:
This position requires strong communication skills, critical thinking, and the ability to manage multiple patients in a fast-paced environment. Experience in case management, discharge planning, and patient care coordination is essential.

Requirements & Qualifications:

  • Active RN license
  • BLS certification
  • Case management and care coordination experience
  • Discharge planning experience
  • Ability to work remotely with reliable home internet
  • Knowledge of utilization review and insurance processes (preferred)
