Job Title: RN Care Manager – Inpatient
Location: Summit, NJ 
Department: Overlook Case Management
Contract length: 8 weeks

Schedule: Monday – Friday, 8:00 AM – 4:00 PM
Hours: 37.5 hours per week (7.5 hours/day)
 

Key Responsibilities

• Develop comprehensive case management plans by reviewing clinical findings, diagnostic reports, and patient assessments.
• Collaborate with physicians and interdisciplinary teams to align care with the patient’s treatment plan.
• Identify high-risk patients requiring psychosocial intervention and refer to Social Services when appropriate.
• Conduct assessments and implement strategies to reduce hospital readmissions.
• Develop and document transition of care and discharge plans in collaboration with the care team.
• Monitor patient length of stay (LOS) and participate in Care Rounds to ensure efficient care progression.
• Communicate financial or payer-related concerns impacting discharge planning.
• Coordinate care transitions between patients, families, physicians, and care teams.
• Maintain knowledge of CMS, Medicare regulations, Observation Status rules, and the 2-Midnight Rule.
• Provide education and resources to patients, families, and care teams to support safe transitions of care.
• Identify potential quality or risk management issues and escalate appropriately.

Required Skills & Experience

• Excellent communication, negotiation, and conflict resolution skills.
• Strong data analysis and computer proficiency.
• Knowledge of state and federal regulatory standards related to case management.
• Experience in clinical performance improvement or rapid cycle change initiatives.
• Prior clinical experience managing hospitalized patients.

Compensation

• Gross Weekly Pay: $2,545
• Weekly Stipend (Meals & Lodging): $1,533
• Estimated W2 Hourly Rate: $27/hr
