Description

Pediatric RN Case Manager providing in-home hospice and concurrent care services for patients birth–21 years. Responsible for care coordination, skilled nursing assessments, symptom management, and family education.

Active CA RN License Minimum 2 years pediatric experience (PICU, NICU, pediatric acute care, oncology, and/or hospice) Hospice or case management experience preferred BLS required Reliable transportation Valid driver’s license Current auto insurance with acceptable coverage Current vehicle registration
