Submission Requirements
· Must have a minimum of 24 months CT Technology experience (with recent experience in last 12 months) - [REQUIRED]
· Life support certifications must be from AHA, ARC, or ASHI - [REQUIRED]
· No hits on state license to practice - [REQUIRED] [DOCUMENT]
· RTO cannot exceed 7 days, No RTO during the first week of assignment - [REQUIRED]
· Candidate driver’s license to verify residency - [REQUIRED]
· Flu Vaccination: Medical and/or religious exemption forms available upon request. Please include vaccination status in submission notes.
· Travel experience - [HIGHLY PREFERRED]

· Active BLS certification from approved vendor [REQUIRED] [DOCUMENT]
· Flu vaccination [REQUIRED] [DOCUMENT]
· Driver's License [REQUIRED] [DOCUMENT]

Shift Notes:
7PM-730AM or 430PM-3AM/36 and 40 hour schedule/Banner is allowed to cancel 1 shift per week/Must be willing to float to units where experienced/Dual campus requirement
