Job Summary:
We are seeking an experienced Home Health Registered Nurse (RN) to provide high-quality patient care across Los Angeles County. The RN will manage a high-volume caseload, perform patient assessments, and ensure accurate documentation, including OASIS, in a fast-paced home health setting.

Key Responsibilities

• Provide direct patient care in home health settings.
• Complete OASIS assessments and documentation accurately.
• Manage a caseload of 5–6 patients per day.
• Develop and implement individualized care plans.
• Monitor patient progress and coordinate care with the healthcare team.
• Educate patients and families on treatment plans and care.
• Ensure compliance with home health regulations and documentation standards.

Schedule & Work Details

• Start Time: 8:00 AM (strict adherence required)
• Weekend Requirement: Every 4 weeks
• On-Call: Required
• Coverage Area: Los Angeles County (5–20 mile radius)
• Mileage Reimbursement: IRS standard rate
• Pay Model: Hourly (not per visit)

Required Experience

• Minimum 2 years of Home Health experience (high-volume caseload required)
• Previous Home Health travel experience required (no first-time travelers)
• Recent OASIS experience required (mandatory)
• Must pass an OASIS exam prior to interview

Required Certifications & Licensure

• Active RN License (must be active at submission)
• BLS Certification
• Flu Vaccine required (no exemptions)

Additional Information

• Locals Accepted: Yes (no radius restriction)
• Strong familiarity with OASIS E11, G-Section, GG Section, and M18 required
