Description

Job Summary:
We are seeking an experienced RN Case Manager to support patient care coordination and discharge planning in an acute care hospital setting. The RN will work closely with interdisciplinary teams to ensure efficient, patient-centered care and smooth transitions throughout the continuum of care.

Key Responsibilities

  • Coordinate and manage patient care plans and discharge planning.
  • Collaborate with physicians, nurses, and multidisciplinary teams to ensure timely and effective care delivery.
  • Assess patient needs and develop appropriate discharge plans.
  • Facilitate communication between patients, families, and healthcare providers.
  • Ensure compliance with hospital policies and case management standards.
  • Maintain accurate and timely documentation.

Schedule & Work Details

  • Start Date: ASAP
  • Shift: Day shift
  • Patient Ratio: 1:20
  • Weekend Requirement: Every other weekend (must be available for holidays)
  • Floating: Not required

Required Experience

  • Minimum 2 years of acute care case management / discharge planning experience
  • First-time travelers accepted (must be properly screened)

Required Certifications & Licensure

  • Active California RN License
    • Pending license accepted but must be active by start date

Additional Information

  • Facility Type: Acute Care Hospital
  • Dress Code: Business attire or scrubs
  • RTO: Maximum 7 days (must be disclosed at submission)
