RN Case Manager – St. David’s Main Campus
Location: 919 E 32nd St, Austin, TX 78705 Units: ICU, Observation, Telemetry, PCU, Neuro, Ortho (assignment varies) Positions Available: 2 Start Date: ASAP Schedule: 8:30 AM–5:00 PM Weekends: Rotating every other weekend On‑Call: None
Unit Overview
- Beds: Varies by unit
- Team Composition: 15 RNs, 15 Social Workers
- Day Shift Staffing: 18 staff members (mixed disciplines)
- Night Shift: Not applicable
- Patient Ratio: 1 RN Case Manager : 30 patients
- Equipment: Standard office and computer equipment
- EHR Systems: Meditech, Midas
Position Summary
The RN Case Manager supports patient care coordination across multiple inpatient units, ensuring safe and efficient discharge planning, care progression, and communication with patients, families, and providers.
Required Qualifications
Candidates must have the following to be considered:
- Experience with Meditech or prior HCA system experience
- Minimum 1 year of bedside case management
- Strong knowledge of post‑acute care pathways
Preferred Qualifications
Candidates with these skills will be prioritized:
- Meditech/Midas or HCA experience
- Throughput experience
Key Responsibilities
- Lead and coordinate discharge planning
- Facilitate care progression and interdisciplinary communication
- Conduct difficult conversations with patients and families
- Collaborate with physicians on care plans
