- 2 years with 1 year MS in last 5yrs,

- BLS, CPI (offered onsite)

- Must float PRN, 3 facilities w/in 24 mi of main: Mesa, Roosevelt, Maryvale;

- MUST be willing to float down to BHT PRN

- Blackout RTO Dates (if applicable): 5/22-25, 7/3-6, 9/4-7
