Job Description:

***MUST have Hospital Case Management experience! EPIC experience REQUIRED! ***

SCHEDULE: 3-12 hour shifts a week! This is a full-time day position with weekend rotation, no call.

GENERAL OBJECTIVES:

Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum.

QUALIFICATIONS:

·Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program
· State licensure as a Registered Nurse (RN).
· Minimum of 3 year's RN Case Management Experience is required.
Flu Vaccine required - no exemptions.
