Swedish Cherry Hill is a specialty hospital focused on neuro and cardiac patients and serves as a Comprehensive Stroke Center. The team provides high-level care coordination in an inpatient acute care environment.

Travelers may float to other campuses including:

• Swedish Issaquah
• Swedish First Hill
• Swedish Ballard

Job Description

We are seeking an experienced RN Case Manager with inpatient acute care experience to join a collaborative care coordination team. The RN Case Manager will work closely with physicians, nurses, and interdisciplinary teams to coordinate patient care and discharge planning.

Schedule & Details

• Shift: 5x8 Days (08:00 – 16:30)
• Weekend Requirement: Minimum 2 weekend days per month
• Floating: Required to other Swedish campuses when needed

Requirements

• WA RN License (Required)
• BLS Certification (Required)
• 2 Years Experience Preferred
• First-Time Travelers Accepted (with inpatient acute care experience)
• Epic Experience Required
• Case Management Experience Required
