Role: RN Case Manager – Home Health/Hospice
Schedule: Full-Time, Days
Locations: Bradford, Randolph, Springfield, White River Junction (VT) & Lebanon (NH)

Overview:

Provide patient-centered care in home health and hospice settings
Work independently while collaborating with an interdisciplinary care team
Flexible schedule supporting work-life balance

Responsibilities:

Assess, evaluate, and manage patient care plans
Coordinate care with physicians, caregivers, and care teams
Monitor patient progress and communicate changes
Educate patients and families on self-care, medications, and treatment
Maintain accurate documentation and participate in case conferences

Requirements:

Active RN license in VT & NH or compact license
Graduate of accredited nursing program
Minimum 2 years Home Health/Hospice experience
