This is a field based home health assignment requiring comfort with independent visits, OASIS documentation, and HCHB software. Ideal for seasoned home health RN's who enjoy autonomy, structured schedules, and patient focused care.

Minimum Requirements:

• Current Registered Nurse License for the state in which the nurse practices
• RECENT home health experience required
• OASIS documentation experience required
• HCHB (homecare homebased) software experience required
• BLS
• Ability to manage and independent home health caseload
• Comfortable traveling between patient homes.
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age
