Description

First-Time Travelers Accepted (with inpatient hospital/acute care experience)

Hours: 08:00–16:30, Monday–Friday block
Holidays: Required as scheduled
Weekends: Not required (optional shifts available)
On-Call: Not required
Floating: Required between Bel Air & Aberdeen facilities (10-mile radius)
Minimum 3 years of Case Management experience
Must have Inpatient Case Management (INPT CM) experience
EPIC experience preferred
Maryland RN License – REQUIRED
BLS – REQUIRED

Job Responsibilities

Manage an average daily census of ~15 cases
Provide and coordinate:
- Medical home care planning
- Post-acute placement
- Social services coordination
- Wraparound support for safe and appropriate discharges
Utilize EPIC for documentation
Collaborate with interdisciplinary teams to support discharge planning and resource needs
