Job Title: RN Care Manager – Home Health
Location: 10123 SE Market St, Portland, OR 97216
Facility: Adventist Health – Portland
Shift: Days | 5x8s (Mon–Fri, 8:00 AM – 5:00 PM)
Duration: 13 Weeks
Start Date: 04/27/2026

PR: $34/hr on W2 + $1640/week Stipend
Weekly Gross Pay: $3000 for 40 hours
Weekly Hours: 40

Job Description:

We are looking for an experienced RN Care Manager (Home Health) to provide high-quality patient care in a home setting. The RN will manage a caseload, coordinate care plans, and collaborate with interdisciplinary teams to ensure optimal patient outcomes. This role requires strong clinical judgment, independence, and effective communication with patients and families.

Requirements:

Active RN License (State of practice)
BLS / CPR Certification
Valid Driver’s License
Reliable vehicle with current insurance
Home Health or Acute Care experience preferred
