Travel Pharmacist – Home Infusion / Clinical Pharmacy (Day Shift, Contract-to-Hire Option)

We are seeking an experienced Clinical Pharmacist to support a home infusion pharmacy team. This role blends clinical oversight, sterile compounding, and operational workflow in a fast-paced, patient-focused environment.

Job Overview

• Specialty: Pharmacist (Home Infusion / Clinical)
• Schedule: Monday–Friday with rotating day shifts
• Shift Options: 8:00 AM – 5:00 PM, 8:30 AM – 5:30 PM, or 9:00 AM – 6:00 PM
• Contract Length: 17 weeks
• Start Date: ASAP
• Guaranteed Hours: 40 hours per week
• Setting: Home Infusion Pharmacy
• Contract Type: Contract-to-Hire opportunity

Responsibilities

• Review and verify medication orders for appropriateness (drug, dose, route, labs)
• Manage new patient starts, refills, and medication reconciliation
• Provide patient counseling and education on infusion therapies
• Monitor lab-dependent therapies and adjust treatment as needed
• Coordinate care with providers, nurses, and interdisciplinary teams
• Perform sterile compounding and ensure compliance with USP standards
• Oversee prescription entry, documentation, and claim adjudication
• Supervise pharmacy technicians and support workflow operations
• Maintain inventory, lot tracking, and medication safety standards
• Ensure timely preparation and delivery of medications

Requirements

• Active Pharmacist license (state-specific, required at submission)
• BLS certification
• Recent experience in home infusion, hospital, or clinical pharmacy setting
• Sterile compounding experience required
• Strong clinical judgment and attention to detail
• Ability to work independently in a fast-paced environment

Additional Details

• Experience with infusion therapies and specialty medications preferred
• Familiarity with pharmacy systems and documentation workflows
• On-call coverage required (evenings and weekends on rotation)
