Role Overview

We are seeking an experienced Physical Therapist (PT) to join our Home Health team serving patients within Solano County, California, with potential territory expansion as needed. The ideal candidate is a licensed clinician with strong clinical judgment, excellent communication skills, and the ability to work independently in the field.

Key Responsibilities

• Perform and prioritize Start of Care (SOC) visits
• Provide clinical supervision to Physical Therapy Assistants (PTAs)
• Submit next-day mandatory reporting for all SOC visits
• Complete all clinical notes and documentation within 48 hours of each visit
• Maintain consistent and timely communication with supervisors, intake teams, and office staff
• Deliver high-quality, patient-centered home health care
• Follow productivity guidelines and scheduled caseload requirements

Requirements

• Active California Physical Therapist License
• Minimum 1 year of Home Health experience
• Strong understanding of home-based care standards and compliance
• Ability to work independently, manage time effectively, and travel within assigned territory

Territory & Schedule

• Primary Territory: Solano County, CA
• May expand to additional areas as needed
• Work Schedule: Saturday–Wednesday, full 40-hour work week

• 3 days total:
  • 1 day in-house
  • 2 days in-field training
