Role Overview

We are seeking a dedicated and skilled Physical Therapist (PT) to provide high-quality rehabilitation services within a Skilled Nursing Facility and Long-Term Care environment. This role is part of a rapid response crisis assignment and requires a clinician who can quickly adapt, deliver effective treatments, and contribute to strong patient outcomes.

Key Responsibilities

• Conduct comprehensive evaluations of residents and develop individualized treatment plans
• Deliver therapeutic interventions to improve mobility, strength, function, and overall quality of life
• Document assessments, progress notes, and care plans according to facility and regulatory standards
• Collaborate with interdisciplinary staff including nursing, therapy teams, and medical providers
• Monitor patient progress and adjust treatment plans as needed
• Educate residents, families, and caregivers on safe mobility and therapy goals
• Ensure compliance with all state and federal regulations

Qualifications

• Active and current state Physical Therapist license
• Minimum 1 year of experience as a Physical Therapist
• Experience in Skilled Nursing Facility (SNF) or Long-Term Care settings preferred
• Strong communication, assessment, and clinical documentation skills
• Ability to work autonomously and within a multidisciplinary care team
