Role Overview
We are seeking a dedicated and skilled Physical Therapist (PT) to provide high-quality rehabilitation services within a Skilled Nursing Facility and Long-Term Care environment. This role is part of a rapid response crisis assignment and requires a clinician who can quickly adapt, deliver effective treatments, and contribute to strong patient outcomes.
Key Responsibilities
- Conduct comprehensive evaluations of residents and develop individualized treatment plans
- Deliver therapeutic interventions to improve mobility, strength, function, and overall quality of life
- Document assessments, progress notes, and care plans according to facility and regulatory standards
- Collaborate with interdisciplinary staff including nursing, therapy teams, and medical providers
- Monitor patient progress and adjust treatment plans as needed
- Educate residents, families, and caregivers on safe mobility and therapy goals
- Ensure compliance with all state and federal regulations
Qualifications
- Active and current state Physical Therapist license
- Minimum 1 year of experience as a Physical Therapist
- Experience in Skilled Nursing Facility (SNF) or Long-Term Care settings preferred
- Strong communication, assessment, and clinical documentation skills
- Ability to work autonomously and within a multidisciplinary care team
