• Number of Beds -45
• Number of Staff – 2-3 CM on this floor
• Type of staff:
  • Day Shift – (example 1 charge, 3 RN, 2 CNA, and 1-3 sitters) days from 8-430 pm , rounds start at 1040
  • Night Shift -
  • May Include weekends coverage
• Patient Ratios – per case manager 18-25 per case manager
• Type of equipment -
• EMR - Meditech, CarePort

Typical hiring profile

Skill Set Most recruited for:  (Travel RNs, local CNAs and sitters)

Must have:  candidates without these skills will not be considered for the role.

• Knowledge of discharge planning, patient assessment , patient family conferences,

Working with physicians on safe discharge plan , working with snf’s home health , hospice, LTACH,

Preferred or nice to have:  candidates with these skills will be considered first.

•  Meditech experience

List typical procedures performed on unit(s):  

• Rounding on patients that meet high risk criteria for discharge planning needs, working with MD and Nurses,
• Arranging medication placement, transportation   

Best personality Fit:

• Person that is detailed oriented and enjoys working with a multi team approach with patients

Can handle stress well, and patient focused
