Description

RN Case Management

Shift: Day Shift ( 8:00 am to 5:00 pm) Monday to Friday

Certification: BLS (AHA)

Must have: candidates without these skills will not be considered for the role.

· Meditech or HCA experience

· Bed side case management experience at least 1 year

· Post acute care knowledge

List typical procedures performed on unit(s):

· Discharge planning and progression

· Hard conversations with family, patients and open discussion with MD for plans of care

Candidate Should be above 50 Miles from the Location Full SSN Required at the time of Submission
