Lead RN Case Management
Facility Name: Mercy Rehabilitation Hospital OKC South
Position Information:
• Specialty: Case Management
• Shift: 8am-4pm
• Required Certifications: Current Registered Nurse or Social Work licensure or Masters or bachelor’s in social work ♦ Certification in Case Management or Rehabilitation preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC
Years of Experience: Minimum of 2 years’ social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred. ♦ Previous leadership or supervisory experience strongly preferred.
• Additional Notes: Assist with discharge planning, utilization review, family conferences, team conferences, and other required patient needs and resources to facilitate a safe discharge.
• Patient Population – 18+, Cardiac, ortho, neuro, debility, etc. General patient population. Inpatient Rehabilitation patients.
Hiring Profile:
• Must Have Skills/Experience: Basic computer knowledge, appropriate communication skills, communicate effectively in English (verbal & written)
• Preferred Skillset/Experience: social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred
• Common Diagnosis/Treatment: CVA, TBI, Ortho, Cardiac, Amputation, Neuro
• Personality Fit: Must be cooperative and have desire to be a team player
• Required Education: Masters or Bachelors in Social Work, Licensed RN preferred
