Description

RN Case Manager

13-week contract | Start: ASAP
Shift: Day (5x8 hours, 8:00 AM – 4:30 PM)

Requirements:

Minimum 2 years of Case Management experience
First-time travelers accepted (with clinical screening)
Experience with EPIC charting preferred

Role:

Coordinate and manage patient care & discharge planning
Collaborate with physicians and interdisciplinary teams
Ensure quality care, documentation, and timely transitions

Additional Info:

Weekend requirement: 2 weekends every 6 weeks
Floating required based on department needs
Locals accepted | Free parking available
No call required
