Description

Case Manager - Days - 40H, no weekends
Will accept RN or SW
INPT CM and EPIC experience
3YR experience required; will accept first-time traveler with inpatient hospital/acute care experience; EPIC charting preferred
Start Date: 05/04/2026
Contract length: 13 weeks
Shift/hours: 5x8 Days, Monday thru Friday block (08:00-16:30)
License/certs required: MD LCSW or LCSW-C license required for SW and RN MD state license required for RN; BLS preferred
Average Daily Census – 15 cases daily
Patient Diagnosis - INPT case management providing and coordinating medical home care, placement & social issues-wrap around type services for safe discharges.
Scrubs/Attire: Business casual

Holidays: required as they fall, as scheduled
Floating: required between Bel Air and Aberdeen facilities as needed (10 mile radius between the facilities)
Weekends: not required, but opportunity for shifts if interested
On-call: not required
