🚨 Now Hiring: RN Case Manager (Contract)
📍 Washington
🏥 MedStar Health – Family Choice DC
📅 Start Date: May 11, 2026 | ⏳ 13-Week Contract | 👥 4 Openings
🩺 About the Role
Join a mission-driven care team supporting Medicaid populations across Washington, DC. As an RN Case Manager, you will play a critical role in coordinating care, developing individualized care plans, and improving outcomes for high-risk and vulnerable patients across all age groups.
💼 Key Responsibilities
- Conduct comprehensive patient assessments via phone and field visits
- Develop and manage individualized care plans (including Social Determinants of Health)
- Coordinate care with hospitals, providers, and community resources
- Perform utilization review, discharge planning, and care coordination
- Ensure documentation meets NCQA and regulatory standards
- Manage caseloads of complex patients (chronic conditions, behavioral health, etc.)
- Utilize EMR systems (GuidingCare/HealthEdge) for ongoing case management
✅ Requirements
- Active RN license (DC required prior to start; pending accepted)
- BLS certification required
- 2+ years of Case Management or relevant clinical experience
- Experience with:
- Care coordination & discharge planning
- InterQual & Milliman guidelines
- CPT, ICD-10, HEDIS, NCQA (preferred)
- Strong communication, organizational, and documentation skills
🏥 Work Environment
- Office-based with potential field visits (2 days/week)
- Business casual dress code
- Collaboration with social services and interdisciplinary teams
- Focus on high-risk populations (e.g., CHF, COPD, diabetes, pregnancy, behavioral health)
🕒 Schedule & Details
- Monday – Friday | 8:00 AM – 4:30 PM
- No weekends or on-call required
- Local candidates preferred (within 60 miles)
- Mileage reimbursement for field visits
