A strong candidate for us would be a RN with HH experience and has had training with OASIS assessments- this will be the bulk of their case load, completing start of care visits, discharges, recertifications, etc. The expected case load of managing patients is 25-30 patients at 40 hours per week. We operate with an EMR called homecare homebase, easy to teach, but experience with that EMR also a plus. 1. Shift Detail (if variable, please list what primary shifts will be covered), list specific times: 2. Are shifts 8s or 12s? 8 hour days M-F 3. Weekend Coverage/Rotation (i.e. every other, every 3rd): None will be required but is available. 4. Holiday Coverage/Rotation: None required 5. Length of Assignment : Up to 20 weeks as approved 6. Weekly guaranteed hours: 40 hours/week 7. Is Overtime available for this location? Yes with prior approval as needed 8. Targeted State Date: We had anticipated to have a traveler start Feb 23rd, however the nurse we need to cover leave for actually went on leave 4 weeks early. So we have an opening immediately. Job Description Summary: Responsible for and provides overall supervision of the assigned service area patients in concert with additional leadership and interacts with health system departments, medical staff and patient/families. Oversee and assist with scheduling nursing staff to specific shifts, taking into account past utilization trends. Job Description: Responsible to implement the nursing process within Facility and the continuum of care. The nursing process is utilized to assess, diagnose, establish a plan with outcome criteria, intervene, evaluate, and document human responses to actual or potential health problems. Collaborates with other inter-professional colleagues, including providers, to plan, implement and evaluate care.
