Description

RN – Case Manager (Acute Care)

Shift: 5x8 Days (08:00 – 16:30)
Duration: 13 Weeks (Start: 05/04/2026)
Schedule: Every other weekend

 

HARDSTOP REQUIREMENTS (Must Have)

  • Minimum 2 years acute care Case Management experience
  • Experience with:
    • Care Coordination
    • Utilization Review
    • Discharge Planning
  • InterQual experience required
  • EPIC EMR experience required
  • California Case Management experience strongly required
  • Experience in:
    • MS / MS-Tele / ICU (and some ER exposure)

Additional Requirements

  • Travel experience required (open to strong first-time travelers)
  • Acute hospital experience (community setting required)
  • Strong knowledge of:
    • Admission criteria & medical necessity
    • Concurrent & continued stay reviews
    • Prior authorizations
    • Discharge planning & DME coordination

Patient Population

  • Adults & Geriatrics
  • Daily Census: 50–55
  • Patient Ratio: ~1:15

Key Responsibilities

  • Perform Utilization Review & Care Coordination
  • Apply InterQual criteria for admission & continued stay
  • Develop and manage plan of care
  • Coordinate discharge planning and patient transitions
  • Work with interdisciplinary teams (social services, etc.)
  • Ensure compliance with CMS, HIPAA, Joint Commission standards
Note: 2-year acute medical Care Manager experience performing both roles of Care Coordination and Utilization Review and discharge planning, Interqual and Epic are all HARDSTOPS
