Description
RN – Case Manager (Acute Care)
Shift: 5x8 Days (08:00 – 16:30)
Duration: 13 Weeks (Start: 05/04/2026)
Schedule: Every other weekend
HARDSTOP REQUIREMENTS (Must Have)
- Minimum 2 years acute care Case Management experience
- Experience with:
- Care Coordination
- Utilization Review
- Discharge Planning
- InterQual experience required
- EPIC EMR experience required
- California Case Management experience strongly required
- Experience in:
- MS / MS-Tele / ICU (and some ER exposure)
Additional Requirements
- Travel experience required (open to strong first-time travelers)
- Acute hospital experience (community setting required)
- Strong knowledge of:
- Admission criteria & medical necessity
- Concurrent & continued stay reviews
- Prior authorizations
- Discharge planning & DME coordination
Patient Population
- Adults & Geriatrics
- Daily Census: 50–55
- Patient Ratio: ~1:15
Key Responsibilities
- Perform Utilization Review & Care Coordination
- Apply InterQual criteria for admission & continued stay
- Develop and manage plan of care
- Coordinate discharge planning and patient transitions
- Work with interdisciplinary teams (social services, etc.)
- Ensure compliance with CMS, HIPAA, Joint Commission standards
