Registered Nurse (RN) – Case Manager

Shift: Day Shift – 5x8 Hours (8:00 AM – 4:30 PM)
Duration: 13 Weeks
Weekend Requirement: Every other weekend (must be available for holidays)
Care Setting: Acute Care Hospital

Job Overview:

We are seeking an experienced RN Case Manager with a strong background in acute care case management and discharge planning. The ideal candidate will be responsible for coordinating patient care, ensuring appropriate resource utilization, and facilitating safe and timely discharges.

Requirements:

• Minimum 2 years of acute care case management/discharge planning experience
• Active RN License
• Travel experience required
• Strong knowledge of Medicare guidelines and regulatory standards
• Experience with care coordination, utilization review, and discharge planning
• Ability to work independently and manage high caseloads

Key Responsibilities:

• Perform comprehensive patient assessments and develop individualized care plans
• Coordinate patient care across interdisciplinary teams
• Manage discharge planning, including SNF placement, DME, and transportation
• Conduct utilization reviews and determine medical necessity using evidence-based guidelines
• Work with payers including Medicare, Medi-Cal, HMOs, and IPAs
• Handle prior authorizations, appeals, and denials
• Facilitate family meetings and participate in treatment planning
• Ensure compliance with CMS, DRG, and HIPAA regulations
• Utilize Epic for documentation and case tracking

Patient & Unit Details:

• Patient Ratio: 1:20
• Case Load: Up to 25 patients
• Patient Population: Neonates to Geriatrics
• Interdisciplinary Support: IV Team, PT, Respiratory, Social Services, Pharmacy, Radiology, and more
