Description

Core Responsibilities:

Perform care coordination and discharge planning
Conduct patient assessments and develop care plans
Coordinate with interdisciplinary teams and lead family meetings
Arrange DME, SNF placement, and transportation (ambulance) independently
Handle cases involving homelessness, mental health, substance abuse, and social services
Ensure compliance with CMS, HIPAA, OSHA, and Joint Commission standards

Skills & Experience:

Appeals & denials, disease management
Needs assessment & plan of care development
Knowledge of HMOs, IPAs, and regulatory guidelines
Strong understanding of protective services, advance directives, and POLST

Important Notes:

This is NOT a Utilization Review role (care coordination focused)
Acute hospital setting
Must be comfortable working independently with high patient load

Acute Care Hospital
Patient Population: Adults & Geriatrics
Daily Census: Up to 25
Dress Code: Turquoise scrubs or business casual

Key Requirements: Minimum 2 years of acute care Care Manager experience (care coordination required) Travel experience required (HARDSTOP) Active California RN License (pending accepted, must be active by start) Epic experience required
