Job Overview
We are seeking a dedicated Case Management RN for a 13-week travel assignment. In this role, you will coordinate patient care, support discharge planning, and collaborate with interdisciplinary teams to ensure efficient and high-quality patient outcomes in a large acute care facility.

Shift & Schedule

• Day shift: 8:00 AM – 4:30 PM
• Rotating weekend coverage required
• No night shifts 

Assignment Details

• Start Date: May 4, 2026
• Duration: 13 weeks
• Facility size: 500 beds
• Department: Case Management
• Team includes Case Managers, Social Workers, Assistants, and Leadership 

Key Responsibilities

• Coordinate patient care and discharge planning
• Collaborate with physicians, nurses, and social workers
• Ensure appropriate utilization of healthcare resources
• Communicate with patients and families regarding care plans
• Maintain accurate documentation in EMR systems 

Required Qualifications

• Active RN license or Social Work (SW) license
• Minimum 1 year of experience
• Strong communication and care coordination skills

Preferred Qualifications

• Case Management or Social Work experience in acute care settings
• Familiarity with Meditech and CarePort systems 

Work Environment & Details

• EMR: Meditech & CarePort Care Management
• Floating required across hospital units as needed
• Business casual dress code
• Collaborative, team-based care model
