Travel Nurse RN - Case Management

Requirements:

• Experience: Minimum 2 years of related experience required
• License: Current RN license in the state of practice required
• Certifications:
  • Basic Life Support (BLS) from the American Heart Association required
  • Certification in Case Management (CCM) or Healthcare Management (CPHM) preferred

Knowledge, Skills, and Abilities (KSAs):

• Proficiency in computers, software, and web-based applications
• Strong verbal and written communication skills with the ability to present information clearly and professionally
• Excellent organizational and time management skills
• Effective conflict resolution skills with sound judgment and decision-making abilities

Key Responsibilities:

• Manage care planning processes in anticipation of patient movement throughout the healthcare system
• Apply care management policies and evidence-based criteria to ensure appropriate levels of care
• Coordinate and manage care for assigned patients based on individual needs
• Develop, implement, and reinforce post-discharge care plans
• Adapt communication and care approach based on patient population needs, including:
  • Respecting patient privacy
  • Adjusting communication style and explanations
  • Obtaining necessary permissions
• Perform additional related duties as assigned

Compliance & Regulatory Requirements:

• Maintain knowledge of and ensure compliance with all applicable federal, state, and local laws, accreditation standards, and regulatory requirements
• Adhere to the organization’s Compliance & Privacy Program and Standards of Conduct
• Report any suspected or known issues related to:
  • Unethical behavior
  • Patient or employee safety
  • Patient privacy
  • Compliance concerns

• Minimal routine exposure to blood, body fluids, or tissue; however, exposure may occur
• May work with patients with communicable diseases, including entering isolation rooms
• Occupational risk for exposure to communicable diseases exists
• Potential exposure to hazardous medications or waste during handling, transport, storage, administration, or disposal
