Description
- Shift: 9:00 AM-5:00 PM
- This is true at home health position.
- Nurse must have experience with providing care in patients home.
- Lyft rides will be available for this borough.
- 4-8 patients a day and every other weekend is required.
- Job Responsibilities:Duties may include but are not limited to:-Assessing, planning, implementing, and evaluating patient care plans in consultation with healthcare professionals.-Evaluating medical information, as well as providing direct and individualized bedside nursing care to pre-and post-surgery patients.-Preparing, administering and recording prescribed medications.-Changing dressings, inserting catheters, and starting IVs.-Adjusting specialized medical equipment as needed, as well as interpreting and recording measurements.-Monitoring patients' vitals and reporting adverse reactions to medications in a timely manner.-Collaborating on discharge planning with the healthcare team.-Answering questions and educating patients and their families on surgical procedures and post-surgery care.-Maintaining and updating patient files, medical charts, and health records.-Adhering to best nursing practices, standards, and protocols.
- Required Skills & Experience:-Acute Facilities: Minimum two (2) years Med/Surg experience in a large acute care hospital setting.
- The most recent experience should be within the past year.-Post Acute Facilities: Minimum two (2) years Med/Surg experience in either Post Acute Facility or acute care hospital setting.
- Preferred Skills & Experience:-EPIC EMR experience.
- Required Home Care Nursing At Home Care
- Required Certification & Licensure:-New York State RN License
- AHA BLS.-AHA ACLS
- Guaranteed Hours: 37.50
